An Imperial authorization form was a type of code used to clear individuals to access certain areas of the Galactic Empire when making outbound flights from certain spaceports, such as Spaceport THX1138.

Between 1 BBY and 0 BBY, the Star Tours-owned Spaceport THX1138, requested that any passengers headed to the Bothan sector used their Imperial authorization forms before boarding.
